DoD Official: New form provided for 2015 federal taxes
Defense Department civilian and military personnel will begin receiving Internal Revenue Service Form 1095 to help them complete health coverage questions on their 2015 federal tax returns, DoD’s principal deputy assistant secretary of defense for health affairs recently told DoD News.
I did not have a 1095B form on the DFAS My Pay site or receive the mailed form. If you have the same concern - call 1-800-642-1386 & for help to get the form mailed to you.
I was informed that the form was mailed to my deceased husband's last address of 30 years ago, I moved from that address about that same time. Sondra Williams
The form I received was addressed to my deceased husband who died in 1970. It shook us up a bit - had my son whose name is same as his dad's open it. No one else I talked to had gotten one. Finally by phone to DEERS I was told it was something they could not change at present, It showed that my husband and I both had government health insurance! Nancy McKenzie
